Our Approach to Supportive and Respite Care

We are committed to ongoing oversight and training for each of our Direct Support Professionals placed with clients. We welcome your input regarding your goals and desired support level. We work to take a team approach to your service, matching you and your support team with the Direct Support Professional that’s right for you. While CSWI does provide SHC/Respite services to clients, at this time, Personal Care services are not available.

Our Caregiving Team

We seek reliable caregivers with robust experience and care backgrounds who are willing to train within our Registered Apprenticeship program and desire to make a long-term commitment to our clients. At Creative Solutions of Wisconsin, our caregivers are invested in a professional career supporting clients with a higher standard of care.

Getting Started with Supportive Home & Respite Care

After you’ve expressed interest in our Supportive Home & Respite Care services, our Intake Coordinator will reach out to you, your guardian, or your care coordinator to discuss your needs, schedule, location, and staff availability. Our Intake Coordinator will work with our Direct Support Program Director to evaluate staff availability and define our next steps for starting services. Once your services are authorized through county, state, or private funding sources like CLTS Waiver, IRIS, or private pay, our Direct Support Program Director and Team Lead assign the best Direct Support Professional to meet your needs.

Tailoring Your Supportive Home & Respite Care Services

At Creative Solutions of Wisconsin, our clients and their client guardians are active participants in their care. We will work with you on goal setting, provide regular team communication, and continued quality assurance throughout our service delivery. You will always have access to our team to discuss any questions or concerns you have and update your service delivery preferences whenever needed. Our goal is to create an atmosphere of open communication to support you in meeting your in-home and community needs.

Program Funding

Funding for Supportive Home & Respite Care Services

Our Intake Coordinator can assist you and your care team with recommendations for procuring funding for our SHC and Respite services. Your main resource for funding will be working with your IRIS Consultant or County Service Coordinator. Private pay options are available and can be discussed at the time of intake.
